O médico ou a instituição não poderá revelar ou exibir dados do prontuário a terceiros, pois estas informações pertencem ao paciente.
Vamos agora à parte prática: Como solicitar seu prontuário médico? Em geral a própria instituição, consultório, consultório disponibiliza o requerimento para este fim. È possível solicitar o prontuário médico dentro do sistema do SUS (sistema único de saúde)
È importante você saber que o direito de receber seu prontuário médico também esta garantido pelo Código do Consumidor conforme o artigo 72 que informa:

Por fim destaca-se que a negativa em fornecer cópia do prontuário médico é passível de indenização. Deste modo é muito importante que o solicitante guarde consigo cópia da solicitação, número do protocolo e data para o recebimento para que em caso de demora ou negativa seja possível usar estes documentos como provas.
